We are currently seeking a compassionate, organized, and professional On-Call Nurse Assessor to support client assessments, care planning, caregiver education, and clinical oversight on a flexible, as-needed basis.
Position Summary
This role is designed for nurses seeking flexible supplemental work on an on-call, part-time basis. Assessments are assigned as client needs arise and may include new client assessments, reassessments, hospital discharge evaluations, caregiver training support, wellness visits, and care conferences.
The Nurse Assessor plays a key role in evaluating client needs to ensure safe, appropriate, and high-quality care services while supporting families and the Family Best Care operations team during important care transitions.
Responsibilities
- Conduct client assessments in homes, hospitals, rehabilitation centers, assisted living facilities, and skilled nursing facilities
- Evaluate client needs related to ADLs, mobility, cognition, fall risk, dementia care, transfers, nutrition, safety, and overall care support
- Develop and update care recommendations and assessment summaries
- Identify changes in condition and communicate concerns appropriately
- Support caregiver education and training related to client care needs
- Participate in care conferences and transition planning as needed
- Maintain timely documentation and communication following visits
- Collaborate with scheduling and operations teams to ensure appropriate caregiver placement
- Provide calm, compassionate communication to clients and families
Qualifications
- Active RN or LPN license in Washington State
- Strong assessment and critical thinking skills
- Experience in home care, hospice, geriatrics, skilled nursing, or case management preferred
- Experience working with dementia and medically complex clients preferred
- Strong communication and documentation skills
- Reliable transportation and willingness to travel locally
- Ability to work independently while maintaining communication with the office team

Compensation
- Competitive assessment-based or hourly compensation depending on assignment
- Mileage reimbursement may be available for qualifying visits
